Duties and Responsibilities:

The responsibilities of a Support Worker at Nurses Group Homecare include:

· Prioritizing service users' wellbeing and incorporating their wishes and preferences into care delivery.

· Contributing to the efficient functioning of the service.

· Assisting service users in maintaining connections with the local community.

· Following care plans and ensuring accurate documentation of service users' needs.

· Informing the office of any changes in service users' requirements.

· Promoting and safeguarding the welfare of individuals supported.

· Building rapport with patients, families, colleagues, and healthcare facility staff.

· Serving as a role model for other carers and representing the service positively.

· Adhering to organizational policies, procedures, and fundamental standards.

· Updating daily records for consistent, continuous care.

Job Description:

Nurses Group Homecare are expanding and are recruiting for an experienced Care Coordinator to become part of a small and very friendly Homecare provider based in Yeovil. We provide support to individuals in their homes with activities of daily life that they need some support with.
The Care Coordinator will work alongside the management team, preparing weekly rotas and ad-hoc changes to existing rotas, ensuring all service users receive regular carers at their requested time and that the highest standards of care are maintained.
Ensure that our team of support staff are providing practical assistance and support to our clients enabling them to maintain their independence
Liaise with representatives of organizations involved in the social network of our clients
Comply with all policies of the organization, as appropriate.
Ensure that the day-to-day care services are operated in accordance with health & safety legislation and current statutory regulation guidance on best practice.
The successful Care Coordinator will have:
NVQ Level 3 in Care, Good IT skills and competency in Microsoft Office, Good verbal and written English skills, good leadership and organizational skills, good time management, able to work alternative weekends ,Full Clean Driving License, and Experience in a similar role within Domiciliary Care.
Salary :
£13.50/h weekdays
£14.50/h weekends

Job Title: Field Care Supervisor (Junior role)

Location: Yeovil

Hours: 37.5 h/week (10 h/office based +27.5h/ field work)

Salary: Starting at £ 24,375 depending on the experience of the candidate

Contract type: full time

Internal Relationships

· Accountable to the Branch Manager

· Clinical Supervision will be provided by the Senior Care Co-ordinator/ Registered Branch

Manager

· Close working team relationship with all other care workers and other Nurses Group Homecare office staff

External Relationships

· Clients

· Families, relatives, and carers

· GPs/District Nurses

· Multi-disciplinary teams

Job Purpose:

· Our Field Supervisors are ambassadors for Nurses Group Homecare. They take responsibility for ensuring our customers are receiving the care and support they need and deserve.

· Working with Multi - Disciplinary teams regarding support and equipment.

· Inducting new workers and providing direct training to both new and established carers, regular spot checks of care staff to ensure safe, caring, responsive and effective care .

· Providing flexible care support to cover sickness and holiday cover to clients in the field or starting a new package of care whilst a suitable carer is found to avoid delay in support starting.

· Being part of the out of hours On Call support system on a rota basis.

Main Duties:

General Standards

· To act in a professional manner

· To adhere to the Nurses Group and Local Authority policies and procedures and relevant contract specifications

· Treat with complete confidentiality, all patient information of which you have knowledge

· To report any area of concern to the Branch Manager, and Continuing Care Case Manager or Team Leader

Quality Assurance

· Carrying out spot checks to monitor the quality of care provided by care workers and

feeding this back to Senior Care Co-Ordinator/ Registered Care Manager

· Supporting and monitoring the implementation of quality assurance processes and agency

policies and procedures

· Support carers concerns around Health and Safety in the Community

Care and Support

· To provide telephone support to clients and care staff

· Overseeing the delivery of care and support. Providing direct care as set out within the care workers job description, when care workers are unavailable, supporting new clients with on boarding a service
